91_SB0764eng SB764 Engrossed LRB9106030JSpc 1 AN ACT to amend the Public Utilities Act by changing 2 Section 13-704. 3 Be it enacted by the People of the State of Illinois, 4 represented in the General Assembly: 5 Section 5. The Public Utilities Act is amended by 6 changing Section 13-704 as follows: 7 (220 ILCS 5/13-704) (from Ch. 111 2/3, par. 13-704) 8 (Section scheduled to be repealed on July 1, 2001.) 9 Sec. 13-704. Billing statement; contents. Each page of 10 a billing statement which sets forth charges assessed against 11 a customer by a telecommunications carrier for 12 telecommunications service shall reflect the telephone number 13 or customer account number to which the charges are being 14 billed. The billing statement shall also contain a separate 15 bill identifying the amount charged as an infrastructure 16 maintenance fee. The billing statement shall also disclose 17 the name and address of each telecommunications carrier or 18 company providing a service for which a charge is billed by 19 the billing statement. The billing statement shall also 20 disclose a toll-free telephone number that will be answered 21 by an operator during normal business hours for each 22 telecommunications carrier or company providing services 23 billed on the billing statement. 24 (Source: P.A. 90-154, eff. 1-1-98.)
