1 AMENDMENT TO SENATE BILL 764
2 AMENDMENT NO. . Amend Senate Bill 764 by replacing
3 the title with the following:
4 "AN ACT to amend the Public Utilities Act by changing
5 Section 13-704."; and
6 by replacing everything after the enacting clause with the
7 following:
8 "Section 5. The Public Utilities Act is amended by
9 changing Section 13-704 as follows:
10 (220 ILCS 5/13-704) (from Ch. 111 2/3, par. 13-704)
11 (Section scheduled to be repealed on July 1, 2001.)
12 Sec. 13-704. Billing statement; contents. Each page of
13 a billing statement which sets forth charges assessed against
14 a customer by a telecommunications carrier for
15 telecommunications service shall reflect the telephone number
16 or customer account number to which the charges are being
17 billed. The billing statement shall also contain a separate
18 bill identifying the amount charged as an infrastructure
19 maintenance fee. The billing statement shall also disclose
20 the name and address of each telecommunications carrier or
21 company providing a service for which a charge is billed by

1 the billing statement. The billing statement shall also
2 disclose a toll-free telephone number that will be answered
3 by an operator during normal business hours for each
4 telecommunications carrier or company providing services
5 billed on the billing statement.
