1 AMENDMENT TO HOUSE BILL 5659
2 AMENDMENT NO. . Amend House Bill 5659 on page 1, by
3 replacing line 18 with the following:
4 "authorized user if a health care provider administers
5 immunizations at that facility, school, or institution."; and
6 on page 1, lines 22 and 23, by replacing "nurse, or other
7 health professional" with "or nurse"; and
8 on page 4, line 3, before the period, by inserting the
9 following:
10 ", unless the child's parent or guardian has elected not to
11 participate in the registry, or to participate only to a
12 limited extent, as provided in Section 2.10"; and
13 on page 4, lines 7 and 11, after "information" each time it
14 appears, by inserting ", if known,"; and
15 on page 4, line 25, by replacing "upon written request," with
16 "only upon written request and only"; and
17 on page 4, line 27, after the period, by inserting the
18 following:
19 "A health care provider who has provided immunization
20 services to a patient shall release immunization information
21 concerning those services only to the patient or to the

1 patient's parent or legal guardian and only upon that
2 person's request, but the request need not be written."; and
3 on page 5, line 10, after "about", by inserting "(i)"; and
4 on page 5, line 12, before the period, by inserting the
5 following:
6 "and (ii) the parent or guardian's right to refuse to
7 participate in the registry and the right to delete or limit
8 information in the registry as provided in this Section and
9 in Section 2.15"; and
10 on page 5, line 18, after "from", by inserting "the parent or
11 guardian of a child aged 18 years or under or from"; and
12 on page 6, line 4, after the period, by inserting the
13 following:
14 "The reports may not include any information that could be
15 used to identify a patient or a patient's parent or
16 guardian."; and
17 on page 6, line 6, before the period, by inserting ", subject
18 to the consent of the patient's parent or guardian"; and
19 on page 6, line 19, after the period, by inserting the
20 following:
21 "The Department shall comply with all such requests.".
