1 AMENDMENT TO HOUSE BILL 3107
2 AMENDMENT NO. . Amend House Bill 3107 on page 1,
3 line 15 by inserting after "facility" the following:
4 "; and the by-laws and organizational chart of the health
5 care facility"; and
6 on page 1, line 21 by inserting after "year" the following:
7 ", except that the first report filed under this Section
8 shall include the information for the corporation's past 3
9 fiscal years" and
10 on page 2, by inserting after line 10 the following:
11 "(7) Patient statistics for the past fiscal year
12 and patient projections for the next fiscal year
13 including patient visits, admissions, emergency room
14 visits, clinical visits, and visits to each department of
15 the hospital, admissions to nursing care, or visits by
16 affiliated home health care providers, except that the
17 first report filed under this Section shall include
18 patient statistics for the past 3 fiscal years.
19 (8) A list of pending or adjudicated citations,
20 violations, or charges against the health care facility
21 brought by any governmental agency or accrediting agency
22 and the status or disposition of each matter with regard
23 to patient care and management of charitable assets for

1 the past fiscal year, except that the first report filed
2 under this Section shall cover the past 3 fiscal years.
3 (9) A description of bad debt owed to the health
4 care facility for which payment was anticipated and not
5 received during the past fiscal year, except that the
6 first report filed under this Section shall cover the
7 past 3 fiscal years.
8 Section 99. This Act takes effect upon becoming law.".
