1 AN ACT to amend the General Not For Profit Corporation
2 Act of 1986 by adding Section 114.15.
3 Be it enacted by the People of the State of Illinois,
4 represented in the General Assembly:
5 Section 5. The General Not For Profit Corporation Act of
6 1986 is amended by adding Section 114.15 as follows:
7 (801 ILCS 105/114.15 new)
8 Sec. 114.15. Information to Attorney General. Each
9 domestic corporation organized under this Act, and each
10 foreign corporation authorized under this Act to conduct
11 affairs in this State, shall file with the Office of the
12 Attorney General, on or before July 1 of each year, the
13 following information:
14 (a) The name of the corporation.
15 (b) The address, including street and number, or rural
16 route number, of its registered office in this State, and the
17 name of its registered agent at such address.
18 (c) The address, including street and number, if any, of
19 its principal office.
20 (d) The names and respective residential addresses,
21 including street and number, or rural route number, of its
22 directors and officers.
23 (e) A brief statement of the character of the affairs
24 which the corporation is actually conducting from among the
25 purposes authorized in Section 103.05 of this Act.
26 (f) The financial size of the corporation.
27 The information shall be provided on forms prescribed and
28 furnished by the Attorney General. The information shall be
29 provided by any authorized officer of the corporation and
30 verified by him or her, or, if the corporation is in the
31 hands of a receiver or trustee, it shall be provided on

1 behalf of the corporation and verified by the receiver or
2 trustee.
3 The information provided under this Section shall be made
4 available by the Attorney General for public inspection and
5 copying.
